Harbor Light Hospice provides its array of services through a team of professionals and volunteers who are physician directed and nurse coordinated. Hospice works because the team works with a patient, their family and caregivers, and the patient’s physician to develop a Plan of Treatment and an Interdisciplinary Plan of Care that coordinates the delivery of that care from the medicine, supplies and equipment to the Nurse, Nursing Assistant, Social Worker, Chaplain, and Volunteer visits.
